How does it benefit our employees?
Health Assessment programs give employees valuable insight into their health. During a private assessment and coaching session with a health technician, individuals will learn their cholesterol, blood sugar and triglyceride levels. They will be asked to complete a lifestyle questionnaire while their risk factors—weight, blood pressure and waist-to-hip ratio—are assessed. The final, detailed report will give each employee the information they need to improve their health, eliminate risks and live an energetic, engaged life.
How does it benefit us as employers?
For employers, we provide aggregate reporting that will reveal employees’ top health risks. This detailed health assessment will help you allocate your company’s health and wellness dollars and provide information year-after-year to identify trends in health status and adjust wellness strategies to fit them. Aggregate reporting is confidential with no individual results or identifiers provided.
Health Assessment Delivery Options
The Health Assessment program can be delivered in a variety of ways to suit your objectives and budget. Health information kits and marketing support can be added to any of the program options to help you reach your participation goals.
Online Questionnaire Only
Employers first distribute the questionnaire to employees via email. This extremely cost-effective and efficient method gives employees access to the lifestyle questionnaire while collecting information to help you direct wellness planning. It also gives employees access to a personal health report that will outline any risks. Personal and aggregate reporting is available.
Online Questionnaire + Coaching
Upon completion of the questionnaire, an Apex health professional will meet with employees one-on-one to discuss their answers. Coaching is based on the results from the online questionnaire and personal health report.
Blood Analysis + Coaching
Each employee will meet with an Apex Health Technician who will collect a finger-prick blood sample to be analyzed for various biometrics. Employees will then have a one-on-one coaching session to discuss the results. The full consultation, including blood sample and analysis, only takes 15-20 minutes per employee. Aggregate companywide reporting is not available.
Full Service
Employees have access to an online lifestyle questionnaire, blood analysis and coaching with an Apex health professional. Personal and aggregate reporting is available.
Full Service + Health Fair
Employees have access to an online lifestyle questionnaire, blood analysis and coaching with an Apex health professional. Personal and aggregate reporting is available.
Additionally, Apex Wellness, together with employers, will organize a Health Fair to promote the company’s new health initiative and help support other wellness resources and activities available to employees.
